+ Enrichment + Fun = A+ Summer School. Grades K-12
Dear Parent:
The three-month long summer is almost here, but learning cannot take a
break. Please read the following information or call us at 650-286-1688. You may avoid being placed on waiting list
by reserving seats: Just download and fill out the registration form and
mail it back with $60 nonrefundable registration fee. Tuition and book fee
are due two weeks before your child starts with us.
Summer vacation is here, but learning doesn't take a break. This
Summer School program provides half-day or full-day opportunities that are
academically focused but oriented for fun. Based on grade and skill
levels, students are placed in classes taught by experienced, degreed
teachers. In the morning the half-day and full day program includes
Reading
, Writing, Math, Science/Social Studies, Study/Test-taking Skills, World
Study, and ESL. These classes help students review and consolidate what
they have learned and prepare them for the challenges of next year. In the
afternoon, the full-day program shifts into indoor and outdoor activities
including: swimming, library research, supervised sports and games, arts
and crafts, and field trips. Students can enroll for 3 to 10 weeks,
consecutively or not, in either the half or full-day program. Before and
after program hours are provided for an additional fee of $50 per week for

Agreement:
I
approve this application and certify that the applicant(s) is(are)
capable of A Plus in/out door activities. I agree to pay the the
fees and minimum 3 or up to 9 week¡¯s tuition prior
to the start of the sessions noted above, and I understand that failure to do so will result in forfeiture of
the nonrefundable registration fee and my child¡¯s
enrollment. I agree to pay $10 late fee and/or $20 fee for a
returned check whichever happens. I also understand that no refund
is given if a child withdraws for any reason before completing the
number of week(s) s/he registers or is dismissed because of
disruptive behavior as determined by A Plus Program Director.
I
grant permission for the applicant to participate in all in-out
door activities including in-class games, park sports, field trips
by van/car. In case of accident or illness, A Plus is authorized
to secure emergency medical treatment. Prudent attempts will be
made to contact parents/guardians immediately. I authorize A Plus
to have and use photographs, slides, tapes, and written work of
the applicant as may be needed for its public relations program.
I, as the parent or legal guardian,
hereby assume full responsibility for any risk of bodily injury,
death or property damage, and hold harmless the A Plus Learning
Center, Inc., its directors, officers, employees, agents and
volunteers from any claims, suits, losses, liabilities, or
related causes of an action for damages. I certify that I have
read, and I understand and agree with all the terms in this
Participation Agreement.
